Lost or Damaged Membership Card

Don’t worry, you can order a replacement membership card for £5.00 by filling out the form below. Once verified, a payment QR code will be emailed to the email address provided. Following successful payment please take a screen shot of the payment notification and send here or email cqac.secretary@gmail.com please include your name and membership number if known.

Please allow 7 days following successful payment for replacement cards to arrive, cards will be sent out by Royal Mail.
